Once you click the link and complete the prospective patient inquiry form, our team will thoughtfully review it to determine if our practice is the right fit to support you. If so, you’ll receive an invitation to the MYIO patient portal, an email with a patient forms packet, and an email offering a pending appointment. We’ll also send instructions on how to send us a copy of your photo ID and the front and back of your insurance card so we can verify your benefit details. Once we receive and review your completed forms and insurance information, we’ll be able to confirm your initial appointment. Please note that all paperwork and insurance documentation must be submitted at least 2 business days before the pending appointment. If the paperwork and insurance is not received by then, the pending appointment will be released and will need to be rescheduled.
A: We are currently in network with Aetna (except CVS Health HMO Bronze S, HMO Silver S, and HMO Gold S), most BCBS (except Blue Local, Healthy Blue, or High Performance Network, and BlueHome with Novant Health), United Healthcare, Optum, Meritain, and TRICARE plans. Please contact your insurance company to ensure your “Mental Health” or “Behavioral Health Benefits” are covered under one of these plans, as these benefits may be subcontracted to a different carrier than your main insurance coverage. Please also be aware that some plans purchased on the Marketplace are also out of network, even though they are through one of our In-Network companies.
A: No, we are not in network with Medicaid or Medicare. Our practice has opted out of the Medicare program. If you have Medicare, please be aware that Medicare will not pay for any service provided by this practice. If you would like to continue with this practice, and you have Medicare, a private contract would need to be established that indicates your agreement with these terms in more detail. If you have insurance coverage other than Medicare, that this office is not in-network with, you can submit a claim to your insurance and possibly be reimbursed for some portion of the visit (based on your out-of-network benefits).
A: We do not provide formal psychological testing for legal, court, or disability determinations.However, many diagnoses — including ADHD, anxiety, and depression — are clinical diagnoses that do not require full psychological testing.
Our documentation is often sufficient to support reasonable accommodations in school or work settings.
Please note we cannot complete evaluations for court cases or disability claims that require formal testing reports.
