The current Self-Pay Rates are as follows:
Comprehensive Intake Evaluation Appointment
90-minute (99204 + 90833) Appointment: $455
Therapy Follow-Up Appointment
60-minute (90837) Appointment: $260
Medication Follow-Up Appointment
20-40 minute (99213-99215) Appointment: $130-$300
I understand that the Initial Evaluation Appointment is 90 minutes.
If I have appointments with two different providers on the same day, I will be responsible for two charges, if applicable, as each is billed as a separate appointment.
I understand that the above rates are estimates for medication management appointments based on default CPT codes. However, final determination is based on the CPT that the provider uses based on the amount of time and complexity of the visit.
I understand that the Self-Pay Rates may change from time to time. I may access the most current Self-Pay Rates at any time via CMHC’s website.
CMHC offers a same-day payment discount of 20% discount. I understand that this discount is not guaranteed. I also understand that if there are problems with my payment at the time of service, the same-day discount will not be applied.
