At Casey Mental Health Collaborative, we provide comprehensive pre-surgical psychological evaluations and support letters for patients preparing for bariatric, plastic/reconstructive, and gender-affirming surgeries. These assessments meet surgical and insurance requirements while honoring each patient’s voice, dignity, and goals.

Bariatric Surgery Evaluations
We explore eating behaviors, emotional readiness, lifestyle, and supports—critical for success after weight-loss surgery.
Plastic & Reconstructive Surgery Evaluations
For procedures where psychological clearance is required (e.g., post-mastectomy reconstruction, body contouring, elective cosmetic surgery), we offer confidential evaluations tailored to surgical protocols.
Gender-Affirming Surgery Evaluations
Grounded in the WPATH Standards of Care, our affirming, trauma–informed evaluations explore gender history, mental health context, informed consent, and readiness. When criteria are met, we provide timely support letters in alignment with gender-affirming care guidelines.
What to Expect
1. Scheduling & Intake
We’ll guide you through a brief intake and collect any documents required by your surgeon or insurance provider.
2. Virtual Evaluation Appointment
You’ll meet with a licensed clinician for a 60–90-minute video session to discuss your history, goals, and preparation for surgery.
3. Written Letter or Report
Within 5–7 business days, you’ll receive a customized evaluation report or letter of support, addressing the specific criteria of your procedure.
